課程概述

This practicum in an acute care environment provides students with direct experience on development of a systematic methodology of identifying patients' needs for health care. The focus of the practicum is on assessment, on risk/risk development and on the subsequent management of selected health problems for a caseload of patients at the emergency room. Critical thinking, clinical reasoning and problem solving are emphasized. Practice and mastery of these skills will occur in preceptor clinical settings and will reflect progressive competency of students. Bi-weekly grand round will focus on cases presented by the students as well as specific topics that relate to management of common health problems in acute care settings. Required for all Nurse Practitioner students in the second year of specialization with 9 clinical hours per week (3 credit hours).

2. Designs therapeutic and nursing interventions for selected health problems.
3. Relates clinical findings and conclusions in a systematic manner to ensure
continuity of care.

nursing and medical colleagues, and others.
C. Functions effectively under stress and time constraints.
D. Participates in educational/professional activities which promote career

課程要求
Guidelines for Evaluation/Grading:

Demonstration of clinical competence on selected patients with admission note and progress notes which reflect appropriate assessment, diagnosis, interventions, and evaluation. Appropriate professional behavior as guided by course objectives.

Times of practice may be flexible depending on your learning needs and the needs of the patient you have chosen to work with. Students must inform faculty of their clinical hours. Students may arrange for observational experiences in special outpatient clinic or other clinical sites as desired. Communication with faculty concerning these arrangements must be made prior to contacting the observational site.

Completion of Five Admission Notes to include:

1) History and Physical: written presentation of data including patient's most recent pertinent laboratory values, findings of diagnostic work-up and patient-specific rationale for medications.
2) Identification of problems (problem list must be clearly identified).
3) Plan of care: development of written care plans supported by current literature (research based preferred). Copies of sources should be appended.
4) Evaluation: how plan was implemented, how your plan followed or differed from the established plan, self-evaluation, reflections on your progress, goals for the following week.

Completion of Daily Progress Notes:

Weeks during which you do not submit an admission note, you are required to hand in a progress note. Demonstration of accountability for your plan (e.g. abbreviated assessment, what has been accomplished and what yet needs to be done or changed). You may use the format used by staff on the particular unit or use problem orientated style (for example, SOAP notes). Copies are to be handed into the faculty on Wednesday at Grand Rounds.

Completion of Study Passport (see Appendix for details):

A NP student passport is similar to a portfolio but may be characterized as a formal list in which students present the marketable skills or learning objectives they have developed or obtained. Each passport item is signed off by an evaluator, usually the faculty or preceptor. The NP Study Passport at the NTU has been distributed to each student at the beginning of NP practicum. It will allow faculty to assess study progress. This is to be brought, complete to your clinical evaluation at the end of semester for review.
